your pet is a permanent identification that cannot be lost. Make sure your veterinarian has your current address and phone number. If someone calls the clinic with your pet's Rabies Tag number, we cannot connect your pet back to you if we cannot reach you. Keep your own records at home current and accurate, too.
The first time you bring your pet into our clinic we give you a yellow file folder to keep all your pet's information in. If you need a file folder, please ask for one the next time you are in.
Some things to make sure you have in the folder are:
- Record of your pet's vital statistics. These are listed on the annual report card Dr. Smits completes when your pet comes in for their annual examination.
- Your pet's Rabies Certificate and make sure your pet has their current Rabies Tag on their collar.
- Maintain the data from other identification tags on your pet such as Microchip information, Pet License, etc.
- Keep record of the color of your pet's collar and any other miscellaneous information.
- Keep a current picture of your pet.
